Responsible for scheduling and accurately tracking medical/dental appointments and communication of information to the Nurse Case Manager.

CP Unlimited of Hudson Valley is currently seeking a DSP Non-Clinical Coordinator to work in our Residential program to provide people with intellectual and developmental disabilities assistance with all medical and dental appointments and to advocate for their health, welfare and safety.
Specific duties include:
Schedules medical and/or dental appointments for people receiving services. Follow up on all recommendations (lab work, radiology, etc.) Ensures medical records are filed/scanned appropriately.
Provides transportation for people receiving services for medical/dental appointments, urgent care, hospital and/or emergency room visits.
Participates in chart reviews and follow up for internal surveys.
Following visits ensures that new prescriptions for medications / treatments are communicated to the nurse and noted in the medication administration record.
Facilitates process to ensure refills are processed to ensure medication supply is available. Reports any concerns/discrepancies to the MCMC coordinator and nurse.
Requirements of the position include:
High School diploma and or GED equivalent
Upon hire must have a valid NYS driver’s license and be within Agency Insurance guidelines
Certifications required within 3 months of hire –AMAP and CPR / BLS
Prior experience working with developmentally disabled individuals minimum of 2 years preferred
Proficient with Smart phones (Apple), Microsoft office and able to learning new software applications
Excellent organizational, oral and written communications skills.
Ability to travel to multiple locations throughout the Putnam and Duchess county area.
Ability to work flexible hours and/or days
